The family claim descent from ancestors who were of old monarchs of all Ireland, and who at the invasion of Henry II. were kings of Leinster. They bore the surname of Mae-Murrough; but in 1171 Donell, son of Dermot Mac-Murrough, acquired that of Caomhanach or Cavanagh, which became hereditary. Donell's sister Eva married Strongbow, Earl of Pembroke, the leader of the English expedition.

Cavanagh came from the name Caomhan. One of the first kings of Ireland is reputed to have come from Spain and adopted the name Caomhan, meaning 'son of Caomh'. He came with several brothers, almost all of whom died on the way. Those who did make it, including Caomhan, had wives that fought, eventually ending with Caomhan killing his brother and becoming sole king of Ireland.
