It may have referred to someone from any of several small places with names containing cas.
Alternatively, the name may be a Latinized form of the Greek surname Doukas. The Doukai were a prominent Byzantine noble family from Paphlagonia, in what is now northern Turkey, especially influential during the later Byzantine period. Many members would later assimilate into the Turkish and Albanian populations.
