It is believed to have been a mixture of fathers surname (Jensen) and his own (Madsen).
Before this time, the surnames of the Jemesen's were based off the patronymic tradition common in Denmark.
Neils older brother, Rasmus Madsen, kept the surname of Madsen but issued only one daughter.
It is unknown if his younger brother, Jens Christian Madsen, changed his name or had issue.
